When do children and teens need vaccinations?

Livonia Family Physicians has tried unsuccessfully to participate in the Vaccine For Children program through the State of Michigan.  Due to several requirements and protocols the the State requires, we will  not be participating in the program.  We understand due to financial reasons, you may not be able to afford the cost of these vaccines.  Although we would recommend vaccination at the time of your visit, since that may not be an option, we recommend that you contact your local health department for the vaccines.  If you live in Wayne County, there are several immunization clinics available to take your children for these vaccines at no charge.  They have appointments available on a scheduled basis as well as walk in visits.  It is recommended to call and schedule an appointment to make the process more efficient.  Please click below for clinic schedules.

FAMILY ACCOUNT INSTRUCTIONS:

 

Please follow the instructions below to create what's called a "Family Account".  This will enable you to link all accounts under one primary account.  If you are a parent who has more than one child, this will be especially helpful:

 

  1. Go to “Create an Account” and fill out information for your first child.
  2. Provide and submit all required information.
  3. You will know you are finished when you reach the confirmation message.  If this is your only child, then you have completed the registration process.

 

For more than one child:

  1. After you fill out the online forms for the first child, go back to “My Patient Page” and click on “Change My Family Account” in the “My Account” box.
  2. To create another account for the second child, click on “Create a Family Member” under “My Family Account.”  Then go through the same process.
  3. Link those 2 accounts by clicking on “Link an Existing Family Member.” 
  4. Repeat this process for every child in your family that will be seen in our office.
  5. The next time you log in, using any of the ID’s/passwords that you created, you will be able to specify which child/patient account you want to bring up by choosing it in the dropdown box where it says “Active Account.”
