Our office is associated with MIMS (Michigan Integrated Management Services) which provides case management services for our patients.  The case management team includes social workers and nurses who are skilled in assisting families with issues regarding care of an elderly family member.  They will help determine your needs and identify solutions and resources for you, including recommendations for assisted living and nursing facilities.  If needed, our case managers can provide direct assistance with placement of a loved one into an assisted living facility or nursing home.  If your loved one is in the hospital and may need rehabilitation services or additional nursing services, the case managers can assist you with these as well.  They also can provide assistance with home care and durable medical equipment needs.

Call the case management voice mail line at 248.522.2810 and leave a message with your name, phone number and a brief message and someone will return your call.  These services are at no cost to you and they will be happy to assist in any services they can.
