How do I make a "Family Account"? 1. Go to “Create an Account” and fill out information for your first child. 2. Provide and submit all required information. 3. You will know you are finished when you reach the confirmation message. If this is your only child, then you have completed the registration process. For more than one child: 4. After you fill out the online forms for the first child, go back to “My Patient Page” and click on “Change My Family Account” in the “My Account” box. 5. To create another account for the second child, click on “Create a Family Member” under “My Family Account.” Then go through the same process. 6. Link those 2 accounts by clicking on “Link an Existing Family Member.” 7. Repeat this process for every child in your family that will be seen in our office. 8. The next time you log in, using any of the ID’s/passwords that you created, you will be able to specify which child/patient account you want to bring up by choosing it in the dropdown box where it says “Active Account.” |
