We participate in a number of different insurance programs.  It is the patient's responsibility to know their insurance coverage.  If you belong to an HMO insurance program such as Blue Care Network or HAP, please discuss with our office staff what our policies are regarding your coverage.

Due to the frequency of insurance changes, we ask to see your insurance card at every visit.  You will be required to pay your copay at every visit at the time you register at the front desk for your appointment.  You will also be asked to update any personal information such as address, phone number and emergency contact at the same time. 

Please remember that our staff are here to serve you and are required to address every patient upon their arrival to the office.  They must have current up to date information in your chart for your provider at the time of your visit.  Please be patient in their responsibilities as well as the other patients in the waiting room.

For families with children who do not have health insurance, please check out the MIChild program through the State of Michigan.  You can access the link below to complete an application.  MIChild is a health insurance program.
