Both of our office locations have evening hours available.  The Newburgh office is open Monday through Thursday until 8 pm.  We do not book appointments until 8 pm.  We book our last appointment at 7 pm to allow our staff to finish their day and complete work assignments so we close the office at 8 pm.  The Five Mile office is open Tuesday and Wednesday evenings and schedule appointments accordingly.  We close the office once a month on the third Friday between 7 and 9 am for our staff meetings to update employees on important matters that pertain to patient care, updates and health matters.  We also close the office once a month on an evening for our providers to meet to discuss medical issues, updates and business information.  Please call ahead if you are stopping at the office to pick up prescriptions, forms, etc. to assure that the office is open.

• Fourth Tuesday of the month 1pm-4pm

      Appointments are scheduled for one hour

 Prescription Refills

It is very important that you do not sign up with your pharmacy for automated prescription refills.  Due to possible changes in your medication that could happen before your renewal, your pharmacy may request a refill of an old medication.  It would be best to request your prescription through our website by creating an account and then requesting your refill online.
